How long does my enrollment last?

The MFS enrollment form provides for a 30-day termination if for any reason you desire to terminate your participation.

Can my affiliated hospitals participate?

Yes – each hospital would fill out a separate enrollment form if they utilize separate tax identification numbers.  For multiple hospitals utilizing a single tax identification number, a single enrollment form is acceptable.

Can my affiliated physicians participate?

Yes – physicians would fill out a separate enrollment form if they utilize a separate tax identification number from your hospital.
